Dual sided 1 page Monste form for D20 systems

Today’s offering is my 2 page Monster form for Old school D20 games.
I made these to make 1 sheet front & back monster pages for 3-ring binder use.
the monster image box is a click box. Once you click it a popup window will allow you to select an image file you choose. The image will scale the image to fit into the box without distortion. The box is 3″ square.
The preview shows the image of the form.
If all you want is a page to print out then just download the preview and you are set at no charge.
if you want to be able to type in your spell’s data then pay a pittance for the form.
If you wish to save your filled in pages I suggest either saving to an image file, renaming the saved filled in  pdf file, or take a snapshot of the form.
 NOTE: TO USE THE FULL FORM ABILITIES YOU NEED EITHER FULL ACROBAT OR ONE OF THE FREE PDF SYSTEMS THAT HANDLE FORMS.
