Please note the interactive PDF works well on desktops but not on mobile devices due to the non-static fields disappearing, for which I am yet to find a workaround. This will be updated as soon as a solution presents itself (when I become more technically gifted – ie. Please don’t hold your breathe lol). 
The simplified  Word document is far more effective if using a mobile or tablet device.
